Policies

Telehealth

  • In order to receive services by Neha Shah Psychiatry LLC, the patient must be located in the state of Ohio. Location will be reviewed and confirmed at the start of each appointment. If a patient is unable to contract for safety and is deemed an imminent risk to themselves or others, the physician has the right to contact local law enforcement to the patient’s confirmed location to ensure safety.
  • All patients are required to use the HIPAA compliant video telehealth platform provided by Neha Shah Psychiatry, LLC.  We understand that occasionally there will be technical difficulties that prevent a good internet connection. Neha Shah Psychiatry, LLC reserves the right to cancel or reschedule visits in which connectivity problems or poor video/audio quality interferes with communication or quality of services provided.  In some cases, the appointment can be conducted via telephone; however, this is a case-by-case decision between the patient and doctor and is not the preferred or required mode of communication for subsequent visits.  
  • Patients are expected to conduct all appointments with the video and audio on and all parties who are participating in the video visit should remain in view of the camera to optimize assessment and treatment.
  • Patients are expected to be in a reasonably quiet and private environment at the time of the appointment.
  • Each session has designated time limits as indicated in the fee schedule. If patients are late for a session, that time is lost from the session.

Payment For Services

  • All fees for services are due at the time of the appointment.
  • A credit card is required on file when scheduling the initial consultation appointment.
  • If the card on file is denied or expired, and Neha Shah Psychiatry LLC is unable to collect payment, then a $25 fee may be assessed.
  • Neha Shah Psychiatry, LLC does not contract with any government, commercial or third-party insurance payers, and thus is considered out-of-network.
  • If you have out-of-network benefits, you may be able to get reimbursement for services provided by Neha Shah Psychiatry, LLC, but it is your responsibility to contact your insurance payer to discuss your options and fulfill any requirements that may be needed for claim submission.
  • Neha Shah Psychiatry, LLC can provide a “superbill” only at a patient’s request. A superbill is an itemized receipt describing services provided.
  • It is the sole responsibility of the patient to submit any claims to the insurance company. Any additional paperwork, documentation, etc is the patient’s responsibility, and requests for additional paperwork/documentation from Neha Shah Psychiatry, LLC may incur additional fees.
  • Neha Shah Psychiatry, LLC does not guarantee insurance reimbursement for services provided. We encourage patients to review their policy in detail and contact their insurance payer with any questions or concerns.
  • If patients have a Health Savings Account (HSA) or a Flexible Spending Account (FSA), it may be utilized for payment or reimbursement as it is an IRS-qualified medical expense. It is the patient’s responsibility to ensure proper documentation is submitted.
Cancellation Policy and Missed/Late Appointments
  • A minimum of 24 hours’ notice is required to cancel or reschedule an appointment. This request can be sent via the portal.
  • If the 24-hour notice is not provided, or if the patient misses the appointment, Neha Shah Psychiatry, LLC may charge the full amount of the patient appointment time that was scheduled.
  • If a patient is more than 15 minutes late to an appointment without notice, then the appointment will be labeled as “No-Show” and the patient will be charged the full amount for the appointment.
  • We understand that emergencies and unexpected events happen. As a courtesy 1 (one) missed appointment fee per year may be waived for established patients. Because there is a significant time commitment required for a new patient evaluation in addition to the actual scheduled appointment time, no missed appointment fees will be waived for late cancellations or missed appointments for new patient evaluations.
  • Neha Shah Psychiatry, LLC reserves the right to discharge any patient from the practice and terminate care for any patient who is repeatedly in violation of the 24 hour cancellation policy.

Confidentiality

  • Everything that takes place in the appointment is strictly confidential and will not be shared with any other individual or party without the patient’s expressed written consent.
  • Confidentiality may be broken in the following cases:
  •  If either child/elder abuse is reported or suspected, or if the patient is/becomes a “vulnerable adult”.
  • If the patient is a child/minor, the parents/guardians are entitled to know the diagnosis, condition and progress of therapy.
  • If the patient poses a “clear and imminent” threat to themselves or someone else, Family members, law enforcement, and/or threatened parties may be notified.
  • If a court subpoenas records.
  • If the patient releases information with a written authorization.
  • When consultation or supervision with another therapist or medical professional is desired in order to provide the best possible therapy. Such discussions will, of course, remain private within the consultation or supervisory relationship.

Emergencies and Acute Care Needs

  • Neha Shah Psychiatry, LLC does not provide 24/7 care or after-hours/weekend coverage.
  • While a small practice can offer some benefits like highly personal and individualized care, it is not a good option for patients or families with high acuity needs such as complex medical comorbidities, frequent hospitalizations, significant substance use issues, or need for significant support outside of regularly scheduled appointments.
  • Any messages sent via the portal will receive a timely response keeping the above in mind.
  • In the event of an emergency, please call 911 or go to the nearest emergency room.
